Insurance

I am an in-network or "preferred provider" with Regence/BCBS, PacificSource/Reliant Behavioral Health, Moda, Lyra, Aetna, Kaiser, Cigna, and Providence. As a preferred provider, a copay or coinsurance amount is collected at each session. The amount is dependent on your plan, but is usually a set fee or percentage.

I am able to bill other plans as an out-of-network provider. At the time of your appointment I will collect the full fee and bill your insurance. After the first insurance payment is received, I will adjust your portion for each session based on what is paid and refund or credit the difference paid for future sessions.

I recommend that you contact your insurance company to determine the following: 

  • Is BreAnna Dupuis, LCSW covered as a preferred provider with my plan or would her services be out-of-network? (Because I am also credentialed with panels through my affiliation at OHSU I recommend clarifying if I am in-network through the Urban Counseling Collective location.)

  • What is my annual deductible and have I met it? Is my deductible waived or do I have to meet deductible before my plan pays for counseling?

  • What is my co-pay (or co-insurance)? If out-of-network, what percentage of out-of-network mental health services does my plan cover?

  • Are there limits on the number of outpatient mental health visits covered?

If you are interested in teletherapy appointments utilizing a HIPAA compliant online video platform please also ask your insurance company if they provide teletherapy benefits and if they require prior authorization to utilize these benefits.


Self pay

Some people choose not to use their health insurance for mental health counseling. In order to utilize insurance for mental health counseling you must meet criteria for a mental health diagnosis which is reported to your health insurance company and becomes part of your medical record. 

Self-pay is an option if you do not have health insurance or do not desire to utilize your health insurance for mental health counseling.

Payment is collected at time of service. I can accept cash, check, all major credit cards and HSA/ FSA debit cards.

If you do not have insurance or you do have insurance but will not be using those benefits (also known as "self-pay"), you have the right to receive a "Good Faith Estimate" explaining how much your medical care will cost. Under the law, health care providers need to give patients who don't have insurance or who are not using insurance an estimate of the bill for medical items and services.

  • You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services.

  • Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item.

  • You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.

  • If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.

  • Make sure to save a copy or picture of your Good Faith Estimate.
